How to Apply for a Colorado Medical Marijuana Card

Before You Apply: Check Eligibility

Qualifying Medical Conditions

Eligible conditions include: cancer, glaucoma, HIV or AIDS, cachexia, persistent muscle spasms, seizures, severe nausea, severe pain, and Post Traumatic Stress Disorder (PTSD).

Adults & Minors

Adults must:

  • be a Colorado resident
  • be 18 or older
  • have a qualifying medical condition

Minors are defined as any patient under the age of 18. All minor patients are required to see two physicians and submit two physician certificates.

  • minor and primary parent are Colorado residents
  • minor has qualifying medical condition
Minor patients with the qualifying condition of Post-Traumatic Stress Disorder (PTSD)
  • Two physicians must diagnose the patient with PTSD
  • One of the physicians must be a board-certified pediatrician, a board-certified family physician, or a board-certified child and adolescent psychiatrist and attest that he or she is part of the patient’s primary care provider team. This physician or psychiatrist should type their attestation in the etiology section of the physician certification.

Required Documents for Application

Adults:

  • A valid Colorado ID or driver’s license (JPEG or PDF copy for online application)
  • Caregiver’s registration ID if you are applying with a caregiver
  • Credit card or bank account and routing number to pay your application processing fee
Parents and legal representatives of minors:
  • Primary parent’s Colorado driver’s license or ID (JPEG or PDF copy for online application)
  • Certified copy of the minor’s state-issued birth certificate
  • Caregiver’s registration ID if parent is not the caregiver
  • Credit card or bank account and routing number to pay your application processing fee
  • Documentation to prove secondary parent status:
    • Secondary parent’s out of state ID if parent lives out of state
    • Certified copy of secondary parent’s death certificate if parent is deceased
    • Copy of court issued sole custody order if secondary parent does not have any custody
  • Minor patient consent form

Legal representatives of adults:

  • Patient’s Colorado driver’s license or ID (JPEG or PDF copy for online application)
  • Legal representative’s Colorado driver’s license or ID
  • Copies of legal representative documentation, medical power of attorney documents or certified court orders
  • Caregiver’s registration ID if the legal representative is not the caregiver
  • Credit card or bank account and routing number to pay your application processing fee

Create An Online Account

Adults:

Create an account here. Choose an easy to remember username and password otherwise your account will be locked after three (3) incorrect attempts. If you do forget your password, click here to reset it. If you forget your username, click here.

Legal Representatives:

  • Create the online account under the primary parent or legal representative’s name using their email and username
  • Have the primary parent or legal representative sign their name on the application. Minors are now allowed to sign their applications
  • Upload a copy of the notarized minor consent form
    • Only parents of minors complete this form
    • If the secondary parent lives in Colorado, make sure he or she signs the form too
    • Minor patient consent form

You are able to correct all information until you add your physician certification to the application.

See Your Physician

Physician qualifications:
Your physician must meet the following requirements to recommend medical marijuana:
  • Physician must be in good standing and licensed to practice in Colorado
  • Physician must be an MD or a DO
  • Physician is registered to submit certifications online
Physician certifications:
Make sure your doctor verifies that all information on your physician certification is correct during your appointment. Physicians are able to make updates to information on physician certifications at any time.

Apply

Watch this video or follow the steps below.

  • Enter patient information
    • If you do not see your name, click “new” to enter your patient information. Your application will be processed as a renewal if you have applied by mail in the past.
    • If you see your name, click on it to start the renewal process.
  • Add physician certification
    • Click the “Physician certification” tab
    • Click “new”
    • Choose your physician certification from the drop-down menu
    • Click “save”
  • Enter cultivation information
    • Click the “Cultivation information” tab
    • Click “new”
    • Select your cultivation option and complete required fields
    • Click “save”
    • You will be able to change your cultivation option once your card is active.
  • Pay
    Use a credit card, debit card, or electronic check.
